Medical Forms:

• Medical Forms 2019 These must be resubmitted each year. If your scout is not attending summer camp, we only need parts A & B on file. If your scout will be attending summer camp or any other trip longer then 72 hours, we also need part C. Please note that part C needs to be signed by a doctor.
